Integrated Health Care

Your Health. Your Health Home.

At Community Alliance, we recognize that those we serve have need for quality physical healthcare as well as mental healthcare. Yet, for too many, obtaining that physical care can prove to be difficult at best.  The reasons are many and varied.  For some, it may seem too hard to find a physician who will see you.  Or maybe the last time you saw a doctor, it didn’t go so well - they seemed hesitate to prescribe medicine for someone who was already on mental health prescriptions, or maybe they didn’t take your physical complaints seriously, attributing your symptoms to your mental illness.

The fact is that mental health and physical health go hand in hand. Recent research indicates that 60% of all adults with a serious mental illness also have at least one other serious, but treatable health condition - conditions like heart disease, high blood pressure, high cholesterol, and diabetes.  By integrating primary and mental health care, we are working to treat the whole person, with the goal of helping you live a longer and healthier life.

Community Alliance now offers on-site medical care at its offices at 4001 Leavenworth Street. The clinic is staffed not only with a full-time psychiatrist and therapist, but also a physician, nurse, wellness coach and personal health navigators.  Because both doctors are on-site, they are able to coordinate medications with each other and are right there under one roof to discuss individual needs together when necessary.  The wellness coach and navigators can help you in making and keeping follow-up appointments, taking your medicines properly, and learning new tools and strategies to enhance your journey towards both physical and mental health.

We invite you to consider coming to Community Alliance for your day-to day medical care as well as your mental health care.

We invite you make Community Alliance your personalized Health Home.
